Each item is understood and tested against the actual codebase first; a suggestion is not implemented merely because a reviewer wrote it. Empty agreement is ruled out, so the response is either a technical acknowledgement or the work itself. When an item is unclear the implementation stops and clarification is requested instead of a partial guess, and reasoned disagreement is expected when the feedback conflicts with a decision already made.
